it happen twice , I felt horrible .

Don't feel bad. My husband dropped our daughter off the bed twice and the couch once. First time she was a week, second time she was sleeping with him on our bed and crawled off and the couch he thought she would just stay and she crawled off it as well. It happens to a lot of parents.
