They just send an appeal form if you don't agree with what they decided. You don't have to fill it out.

I never did that lol I think they send an appeal with the forms incase they deny your request you should ask I got the same thing

An appeal is when you go in to argue a decision they made like if has been denied you can go make your case as to why you need it
