The size usually refers to their legnth from head to butt. So saying the baby is the size of a fist means they are roughly about 4 inches long from head to butt

First baby some women don't show fully till 25 weeks or so. With my son, I didn't pop till I was 27 weeks. Before then I'd have a belly but not one to where I felt like I was pregnant but knew he was in there. And now with my second I'm showing a lot faster.
