I lost my 4 month baby's vaccine card. If I ask nurse would she give me a new one filled out?

Here, they just make you pay for a new one and will fill it out for you since they have what shots were already given on file :)

Wow must be nice to just pay for one.. Here they treat it like a social security card. Even if I forget it she won't get seen or nothing I need it mandatory

They just give it to you here. I live in Los Angeles county as well. I lost it and they just gave me a new one. No charge.
