Struggling with my doctors office

My doctor had suggested reducing my hours and when I called hr they sent me the proper form to have him fill out, yesterday at my check up I provided the form he told me to give it to the assistant at the front office, when I went up to her she told me she would need to check on something and that I would have to pick it up the next day. I called today morning and told them I want to make sure it's ready for pick up since I might have to leave early just to get the form she didn't know how to fill out yesterday 😒 she told me she is working on it and would call me later when it was ready. I then get a cal back from the office it's someone else letting me know it's not going to be ready today that it would take 10 days, & the girl that was helping me is new. I told her I am feeling sick and tired and really need to turn it in by Monday since hr still has to review it. She told me there's nothing that can be done. When I asked if I could make an appointment to see my doctor she said I could do what I want but that it would be pointless since she handles the forma and that the doctor was not going to sit with me to do a form very rude. When I asked if there's any exceptions rush, or anyone else with more authority she said no there isn't that I would have to wait the 10 days. I am so exhausted and tired of the drive idk how I'm

Going to be able to do this for much longer, I don't want to be stressed about missing or coming in late , getting written up when a simple form can help me out. when I asked how many ppl were ahead of me they told me two. I don't see how that takes 10 days . & there's only so much HR can do since they have to have the doctors form. 😩
