Are you re signing the lease you have with these people for another 6months-a year? If yes then you take that to the leasing office and say you want to renew your lease with them. If you don't want to renew your lease you fill that out with the date you are moving out and take it to the leasing office for them to approve the move out date.

it's not an eviction it would not say "residents notice of intent to vacate" it would be a mandatory order of notice of eviction
