Usually for the baby they will do quad screening around 20 weeks to check for birth defects, you basically go in and get your blood taken, they check for Down syndrome, trisomy 18 I believe and a couple more things... But this is a screening it will tell you how likely you are to have a baby with a birth defect, like say 1 in 387. If you had 387 babies the test is saying one of them would have downs

Like everyone else said, ultrasounds can detect some major birth defects as well as congenital heart defects
