I had my doctor write me a script for one and went right to the Medela site. They walk you through the process and I got my pump in less than a week!

Go to your insurance website and follow links under women's health. It will list the medical supply companies your allowed to order from. Then order it. The supply company will bill your insurance and send you the pump
