did you maybe not update income info? and now you owe them some money back? did you call and change anything? if not call them and ask what the paper is for.
no I had called to see if I could change my address && that was it, but they keep on sending me the same thing for the past 3 months. idk why. && when I do calk all they say is to go online. but my mom forgot the password, so I can't. @emmajeremy2627
there should be a forgot password on the website somewhere. hmm if your still receiving benefits maybe its a mistake in the computer so it keeps sending you that form to request a hearing even though you didnt.
it also says at top 'complete form if you do not agree with decision' so yeah if nothings changed i wouldnt worry about it might be a mandatory paper to send out in case for example you dont agree with the amount there giving you or etc...
a hearing request is where your requesting to go in front of the judge and tell them why you disagree with something or say you got a ticket you can request a hearing to say that wasnt me it was someone else, or try and get the ticket lowered...And the judge then makes the decision on what to do(just an example)
It's saying you didn't qualify for a government assistance program (food stamps, tantf ect) and it's asking if you want to pursue trying to obtain it, if so a judge would be willing to hear your case.
if you didn't apply for any of those things you might want to contact them as someone could be trying to pursue it under your name (identity theft)
