Your human resource department should have all the paper work. You can also do some more research and apply got leave under Family Leave Act in California assuming your place of employment meets the criteria
I had to go thru the state disibility, so I did go online because it takes a really long time to hear back via mail. Got my dr note and set up the login. they have forms you fill out and they contact your employer and the employer has to verify before you get anything. Good luck Hun!

nope you just go down to your human resources and they will give you al the paperwork or tell you how to do it online
