You don't have to at all, I didn't want to use them but my daughter decided she wanted them or she wasn't going to sleep at night but that didn't start till she was about 6 months and she only gets them at nap/bed time.

my younger wouldn't take one but I think its cause I breastfed and every time she'd fuss I'd get her to latch to sooth her. pacifiers help prevent SIDS
