the hospital did it for my son when he was born and it came in the mail. maybe ask them if you could use your discharge papers, wrist bands, the card from the hospital crib, and hospital birth certificate. they should take one of those.

Already tried, they won't take any of that..

We did order his card, but the post office didn't recognize his name with our address, so it was destroyed. 😤

they usually accept the crib card until you get that stuff

I'd call the hospital and tell them it got destroyed and to resend the information for you again.. that may be your only option @henrysmommy

I did all the paper work at the hospital when my daughter was born & I got it in the mail like 2-3 weeks

how did you find out they destroyed it? now you have me worried! we moved to a new place so nothing is really associated with our names and address

go to the ssi place and get them to print off a copy saying U request a new card and on that paper will be his ss number on it

I made sure they gave me a copy of my son birth certificate before o left because my case worker need it what insurance you got
