I agree looks like a heat rash...

could he get that just from sweating? he gets pretty warm especially when he sleeps and I noticed it starting after he woke up from his nap and he was sleeping on our leather couch which he sweats a lot when he sleeps there

yes, over sweating is normally what causes it. maybe try having him sleep with a fan on?

My daughter used to get bad heat rashes. I gave her Zyrtec (Benadryl could work too) and it went away.

I've always used benadryl, and put lotion on it. it should clear up (:
