They usually schedule an ultrasound to make sure baby is where he/she should be as confirmation. They will normally use your last cycle to determine how far along you are until they see the ultrasound but really it's all an estimate

I think they don't.. first they use your first day of your last menstruation to tell you how many weeks you have.. Then they ask you some questions and check you down there and they might take some blood out.. I'm not sure but thats what they did on my first appointment. Then at my second app they did that ultrasound and they told me my due date based on the ultrasound.

i wasnt supposed to have an ultrasound. but with my dates of my last period i was supposed to be about 11 weeks and when he checked me he said i felt kinda small.. like 4 weeks off of what we thought so i had to schedule one for the next day.. apparently he said i have steong abdominal muscles cause he couldnt get an accurate feel.. im a few days further along than we originally thought lol but they might give you one if they have reason to suspect you arent where they think you are supposed to be
