you can fill out the forms on your own. the nurse will come in and talk to you personally and then you can tell her. it's not bad at all.

you could always hyphenate her last name. do both of them and then just have her go by your last name. or you could just do yours. tell him when he straightens up you will change it too his. and if he doesnt straighten up then she has your last name still. its not mean at all. he should understand where you are coming from with the situation.
