Do I have to use mine or dad's last name on the birth certificate?

It's completely up to you. if you want to put your last name and not the babys fathers last name then all you have to do is tell them when they come to put the information on the birth certificate

I found out from legal aid, you could use any name, even a made up one.

I'm using my great grandmother's maternal name.

that is all up to you you have to fill the paper out so put whoever name you please to

any you want

I'm putting both because my child middle and First Last name is dedicated to my dad my dad was all cheesing when I told him lol and the actual last name will be my husband

@mamavynom, what state are you in? would I just leave the dad part blank but put different last name that's what I was wanting to do
